Shift Related Forms make it easy for carers to complete required documentation while on shift. These forms are attached to an individual shift, ensuring that any carer assigned to a shift can view and complete the form during their scheduled time.
You can choose to make forms mandatory or optional, giving you flexibility based on the needs of each shift and client. Once assigned, the form will appear automatically when a carer starts a shift.
Bulk Assign
Assigning or removing forms for multiple clients can be done directly from the client list page, saving you time and streamlining your workflow.
Note: This feature is available on the Premium subscription plan.
Navigate to Clients > List from the sidebar menu.
Use the checkboxes to select the clients you’d like to assign or remove forms for. Make sure the form you want to assign has already been created.
In the top-right corner of the screen, click 'Bulk Action', then choose either 'Add Form' or Remove Form, depending on what you want to do.
In the pop-up screen, select the form(s) you'd like to assign or remove. You can also tick the 'Mandatory' checkbox if completion of the form is required.
Use the filters to search by form name, creator, or date created. Don't forget to click 'Save' to apply your changes.
A confirmation pop-up message will appear to summarise your selections. Review the details and click 'Proceed' to complete the action.
The selected forms will now be successfully added to or removed from both the client profiles and the shift records (including future shifts) of the selected clients.
Assign Forms to Clients
Assigning a form to a client will automatically attach the forms to all current and future shifts. Follow these steps to attach a form to a client's profile.
Create a new form from scratch or from a template.
Navigate to the client list section in the Sidebar Menu and select the client you want to assign a form to.
In the client profile, scroll down to the Associated Forms section and click +Attach Form.
Tick the checkbox(es) next to the form(s) you want to attach. You can also tick the Mandatory checkbox if the form must be completed during the shift.
Click Save to attach the form to the client.
Once saved, the form will be visible to any carer rostered on shifts with that client. The form will also be added to all existing and future shifts for that client. If you remove a form from the client’s profile, it will be removed from their shifts as well.
Assign Forms to Individual Shifts
Follow these steps to attach a form to an individual shift. These forms do not carry over to future shifts.
Create a new form from scratch or from a template.
Create a shift in Scheduler and click the Advanced edit button.
Head to the Shift Related Forms section and select Add forms.
From the dropdown menu, select the form or forms you want to include. You can select more than one.
If any of the forms need to be completed during the shift, tick the Mandatory checkbox(es). Click Add to attach the forms to the shift.
Finally, click the Save button at the top right of the page to save your changes.

Can staff members complete shift-related forms via the web app?
Yes. Staff members with any role who are rostered to a shift can complete shift-related forms through the web app.
To do this:
Log in at https://app.shiftcare.com/ using a desktop or tablet browser
Open the relevant shift
Scroll down to the Forms section
Click on the form title
This will open the form in a new tab, where it can be completed and submitted.
