Tracking and Exporting Care Plan Progress

Learn how to track client progress, view goal statistics, and export detailed Care Plan reports as PDFs or CSVs for audits and reviews.

To start tracking and exporting Care Plan reports, please make sure your Care Plans are set up and active. Read this guide for complete details.

Once your Care Plans are active, ShiftCare provides comprehensive tools to track client progress, monitor task completion, and export care plan data and goals for audits or stakeholder reviews.

The Care Plans feature is only available for Premium plan users.


Care Plan Charts and Statistics

When viewing an active Care Plan, you can now see visual charts and statistics that provide immediate insights into progress and completion rates.

Navigate to Client > List > Click on the client's name. Go to the Care Plan tab and click 'View' on the active Care Plan you want to open.

Goals: Select the duration in the dropdown menu. This section displays progress charts over time, showing the Current Score (latest progress percentage), the Total Tasks linked to the goal, and the Average Completion Rate.

Tasks: Shows the Completion Rate (percentage of scheduled tasks completed), alongside the total number of Completed vs. Scheduled instances.


Exporting Care Plans as CSVs

You can export a Care Plan as a detailed CSV document.

  1. Navigate to the active Care Plan's details page and click 'Export'. You can also click the three vertical dots on the right side of the screen and select 'Export as CSV'.

  2. An email will be sent when the file is ready for download. Navigate to Reports > Download Center to download the CSV.

A CSV file will be generated containing:

  • The shift details, including staff and client details.

  • Diagnoses names and goal descriptions.

  • A list of tasks with their descriptions, timestamps, and notes/comments.


Exporting Care Plans as PDFs

You can also export a Care Plan as a formatted PDF document.

  1. Navigate to the active Care Plan's details page, then click the three vertical dots on the right side of the screen. Select 'Export as PDF'.

  2. An email will be sent when the file is ready for download. Navigate to Reports > Download Center to download the PDF.

A PDF file will be generated containing:

  • Your company logo/details, Care Plan dates, and client information (Name, NDIS number, Age, Gender).

  • A breakdown of diagnoses and goals, including descriptions and visual progress charts.

  • A list of tasks with their intended times and completion metrics.


Care Plan and Goal Activity Logs

You can track a comprehensive audit trail of all goal and task updates via the Report Hub. Navigate to Reports > Report Hub > Care Plan & Goal Activity Log.

Tasks Tab: Tracks all task records, including the task name, completion status, log date and time, intended time, notes, associated goals, and the staff member.

Goals Tab: Tracks all progress updates, including the client name, goal name, progress value, log date, the staff member who logged it, and any notes they added.

Click 'Filters and Dates' or the Date Range menu to open the filter cabinet.

Here are the available filters:

  • Date Range

  • Client

  • Logged By (Carer)

  • Goals: Client, Staff Member, Progress Type, Has Notes, Date Range.

  • Tasks: Client, Staff Member, Mandatory status, Completion status, Has Notes, Date Range.

Click 'View Task Adherence Chart' to see a comprehensive chart detailing the status of completed, incomplete, and pending tasks.

Click 'Export' to export the page's data in a CSV file.


Care Plan Progress Report

You can view a comprehensive report of care plan progress across clients by navigating to Reports > Report Hub and clicking 'Care Plan Progress'.

The review columns include:

  • Client Name

  • Plan Name

  • Plan Age: Shows the percentage of the plan duration that has elapsed.

  • Average Score (%): Displays the average progress of all percentage-based goals for the selected period.

  • Median Score (%): Shows the median progress value among all percentage-based goals for the selected period.

  • Lowest Goal (%): Shows the lowest progress value among all percentage-based goals for the selected period.

  • Task Completion (%): Displays the percentage of completed tasks compared to the expected tasks for the selected period.

  • Task Missed (%): Displays the percentage of missed tasks compared to the expected tasks for the selected period.

  • # Focus Areas: The number of Focus Areas present in that care plan.

  • # Goals: The number of Goals present in that care plan.

  • # Tasks: The number of Tasks present in that care plan.

  • Start Date: When the plan was started

  • End Date: When the plan will end.

  • Review Date: When the plan will be reviewed.

Click 'Filters and Dates' or the Date Range box to open the Filter/Criteria window. You can filter care plans by adjusting the date range, moving the sliders for several factors, and adjusting the update days. Tick the 'Show only care plans due for review' box to hide all other care plans from view.

You can sort care plans by Start Date, End Date, and Review Date by clicking the appropriate up or down button.

Reports can be exported for offline review by clicking the blue 'Export' button.

Head to Reports > Download Center to download the CSV file.
